New patio laying and raising garden help needed

Hello all

I am in planning to tackle my garden by building a new patio .

Got couple question i have mixture of concret and dirt where the patio will be laid at difererent heights my idea is to raise the height of the dirt/grass area to same of the concrete area before i lay the patio slabs.

Also the best way to lay the slabs will be with cement?
When i lay the slabs against the house should i put a barrier for moisteure?
My idea is the either use slabs or even make this

Yes raise the base area level with the existing concrete. Don’t forget to raise the inspection chamber. Lay slabs on a sharp sand/cement mix.
