only sugestion would be to remove the whole thing and perhaps let it soak in a descaling solution, sounds like its gunked up internally.

failing that a new one is probably only answer.

hi bruce sleeving of pipe through a wall (internal or external) is good practice, cement can corode copper (given two hundred years), also sleeving can work as a fail safe if a leak should spring it won't fill the gavity.
