You need to rip out the vinyl and let the floor dry out as best as possible, maybe source the damp and take care of that. Then preparation wise.. You need to create a DPM..On a solid floor we use a neoprene primer, a pva primer may work just aswell, but the neoprene is industry standard for flooring...

6mm should be fine in most case's for karndean. Most important that it is fixed with ring shank nails 18mm at regular intervals. Don't use sharp sand .
You can buy an aggregate/small chippings from a local builders or flooring suppliers to reinforce the screed..
It will take a few layers to get it that hight even with an aggregate...
