wthout out looking at it sounds like the friction stays (hinges) are worn or broken you can buy these hinges make sure you get the correct stack hight mesurement from back to front of hinge these are easy to fit by a diyer but you may wish to chat to a repair company

you will need to make sure of the seals and most important the drain holes would feed water into the house so these need to be blocked and new drain holes made so water runs outside

agreed with xjr double glazing will not stop condensation in side house

you need a balance of vetilation and comfort trickle vents wont be enough

goto ebay for hanles they are cheap there i have my own company and find handles on ebay cheaper thn from a supplier

the short answer to that is yes they can be relaced and the cost would depend on the size of glass you will need

please remember if glass is in door or next to door it must be toughend glass
