a pump won't work with a combi as you shouldn't have a cylinder or roof tank anymore, you don't need to increase any pipes with a combi 15mm is the right size, do you have mixer bath taps and if so are they what are called 'telephone style taps'

No you don't need to heat up the water, as long as the water is turned on and the tanks are full it should work, sounds like you need a new pump but check that it's switched on first.

Then change the inlet valve ( ballvalve) to the wc cistern and see if you have cured it

None, water hammer usually comes from a ballvalve in the tank in the loft or a wc cistern

Yes you have to have a tank in the loft a minimum of 1 metre high but as high as possible, if you have good cold water mains pressure a 40 gallon tank will do but measure the hatch opening

Yes you can connect the bathroom to the cold mains and tee off the 15mm hot pipe for the bath
