Loose Toilet Seat Even After Tightening and Replacing Fixings

I realise this will have been covered before, but I'm not sure of the right terminology for finding what I need.

We bought a toilet seat about 6 months ago and it has always been a bit loose. I have tried tightening the bolts underneath, even replacing the fixings that came with the seat for tapered rubber bungs, but still it comes loose. Today I tightened it again, but noticed the circular discs for the adjustable section are tightly in place, and it is the L shaped part that goes through the hinge for the seat and lid that is actually pivoting a bit. I also think that one bolt on the underside is turning with the nut as I try to tighten it.

Can someone advise what I can do? Fed up of nearly having a heart attack every time it shifts when I am sat on the throne!
