concrete arch for back garden wall

I want to try and make a concrete arch for my back garden wall. The arch itself will be a very 'shallow rise' with the centre only been around 1.5 inch about the ends (if that makes sense). I'm assuming that I should use reinforcing rods?

I'd like to make it the same height as a concrete block and the same width as two blocks. it would be the width of a gate opening + extra 1/2 block width each side.

What mix should I use for the concrete? How many rods + how to fit them?
How to make the mold?

Hiya,mix the concrete 4 to 1 and use a couple of 12mm reinforcing bars which you can bend easily to the arc shape,hope this helps
