Making damp proof continuous

I plan to put a 5" thick concrete plinth on top of an existing concrete plinth. Straight onto this plinth will be a timber framed wall. If I place 1200g membrane under the new 5" plinth, how do I make the DPM continuous with the DPC that will sit under the timber frame? If it runs up the outside of the concrete plinth it is going to look ugly and be liable to ripping. Also how do you stand with DPC when I have to puncture it with screws to fix the timber studding down for my walls?
