Hardwood Internal Door Threshold

Hi folks. We have a door between two areas that are not at same level as each other. One area is solid floor the other a suspended wood floor. The difference between the floor is aprox. one inch. The other doors opening into same suspended wood floor area have what appears to be a raised wood threshold from the bare wood floor. We would like to fit a bit of hardwood as the threshold for the problem door. What we need to know is (a) how do you fix the wood on to the wood/concrete join at the threshold and can you have the bit of wood machined to an exact slab thickness ?
