YET MORE DAMP PROBLEMS

My house is early 1900s and over ten years ago I had a concrete floor relaid in the kitchen extension. I know that a membrane was laid to prevent rising damp. At same time I tried to improve the airflow with increased number of vents. I also have tried to maintain the weather proofing weather proofed on outside of the house. I now have serious damp problems in the kitchen, and am trying to resolve. I have just noticed that some of the outside weather-proofing has blown. I'm not sure whether it is that the damp is be coming up via the concrete floor and than working out through the walls or vice versa. I'm not sure how to approach this, but want to get the house back to solid and dry.
