Bridging Two Concrete Slabs Together and how to Get the DPM Right?

Hi There
I have a job on my extension which I seem to be going round the block when coming up with the correct method. I am making two rooms in to one.
I have removed an old dividing wall and also its brick footings which sits in-between two concrete internal floors. Both have been laid on top of the blue DMP as you can see in the picture. I would like to now lay a new mains water pipe in this trench and fill this in with concrete to marry the two floors together. Could someone help me please and suggest a correct and proper method for doing this.
Many Thanks
Ben
