Removing paint on bathroom walls and tiles...Help!

Hi,
I am currently re decorating each room in my house and i am leaving the bathroom until last as i havent a clue of how to sort out this problem.
Walls - When we moved in the walls were painted a yellow/magnolia colour and we painted a light blue over the top. We made sure the wall was clean and that we had bathroom paint (bought from B&Q), we painted two coats however a few weeks later it started to peel in certain areas.
How to i repaint the walls giving there are gaps showing the colour underneath?
How do i stop future peeling after repainting?
Tiles - When we moved in it looked like the bathroom tiles were painted white, over time they have peeled off a little revealing dark blue paint underneath. They now look off white and tired and need to be updated but i dont want to have to take them off and re-tile.
Is they anything i can buy to removed tile paint?
Does anyone have any ideas to fix this?
