Radiator Paint Keeps Cracking and Flaking off

The radiator in my kitchen is just breaking my heart. No matter how regularly I paint it, the paint keeps cracking and flaking off. The process is taking about 6 - 8 months. After this period it's requiring an entire new paint job.
What am I doing wrong?

I've attached some pictures to illustrate the problem. Basically I will gently rub down the damaged paint work with a wire scrub to create a smooth surface and then paint over. Last time I applied 2 coats. But then about 6 months later I'm back to square one.

Does anyone have any ideas/ tips?
