Blocked Cast Iron Down Pipe for Sink and Washing Machine

Hi all,
I live in a ground floor flat. My sink and washing machine waste water pipe runs along the inside wall of my kitchen to join and exterior cast iron down pipe for rain water (and probably everyone else's waste water).
Just outside my kitchen, there is an access hatch attached on this cast iron down pipe (2 square bolts holding it on it looks like, and a tonne of white paint too).
Has anyone opened up this access hatch before? If so, if it cracks or bolts sheer is it the end of the world?
