Repairing a Fence - How to Fit Rail into Existing Concrete Posts?

Hi
I need to replace part of a close board fence which has concrete posts with the rail going straight through a hole in the post (morticed posts I think they're called).
The section I need to replace is part way down the run. How do I get the new rail in? Would be easy enough to slide in if it was at the end but there's no wiggle room in the middle.
Or do I need to find some kind of bracket to slot into the hole and screw the rail to that?
