Sub floor removal

hello
i have just bought my first flat. It is on the ground and has no damp proof course. I have had the property asscessed and all is well apart from some woodworm and uneven timbers.
I have decided to remove the subfloor timbers and put in a concrete floor as the rest of the flat is concrete and it will prevent any further problms later on down the line.
The room is 4meters square and the subfloor space is roughly 50cm deep.
Just would like advice as to how to go about it.. with particular regards to removing subfloor timbers. Can I just cut them out for example?
Where to put the damp proof membrane. Under hardcore or under concrete.
Thanks for your time and any advice welcome
cheers
