Alterations to Roof Where Extension Roof Meets Main Roof

I have an old end terrace house where the main roof is keyed into the chimney and walls of the house. An extension is built on the back of the house with a gable end, and the end A frame seems to be resting on the rafters of the main roof. I would quite like to be able to make space where the roofs meet, for an access door into my proposed loft conversion in the roof of the extension. Is there any way of achieving this? My thoughts were something along the lines of a dormer window like structure in the main roof, allowing for the door into the loft.
