Bathrooms contain a bath or a shower it says nothing about wash basins urinals etc. It could come under swimming pools and other basins but I would not consider a urinal as a fountain? It is normal to put covers over the bulbs so they are not stolen and look nice. If it does have a bath in the restaurant then ceiling height comes into play over 2.25 meters no problem under that Zone 1 and 2 IPX4 or if water jets are used IPX5 although 12 volt SELV would be better option. Also remember switches if is does have bath or shower will be outside the room or on pull cords.
