Replacing toilet with more modern toilet

Hi
Has anyone seen this type of toilet fitting and can I just remove and replace with modern toilet?

Yes you can but you should check your new loo and the distance between the pan outlet and how far it would be from the wall. You may need to fit a different soil adaptor.
