Installing Cooper Single pull combination 274V-Box

I can't seem to get both switch and plug to work at the same time. The instuctions are very short. I'm trying to connect to one power source. I did not remove the separation tab. Any wiring tips

I would ask on a USA forum. This forum is for UK and we don't use that type of socket.
