Yes, you can tile directly onto plaster but you need to make sure your tiles are not too heavy. Gypsum plaster has a maximum weight tolerance of 20kg/m2. Before you tile remove any damages or flaky plaster and fill. You should also apply a tile primer to the plaster first.

Yes I would agree - of the stain is changing then it would indicate damp - through the floor or maybe a window frame. First, you need to stop the damp then you need to remove this damaged plater and replace it. Have you had a dmap specialist come out to assess the stains?

Your picture is too small. It needs to be much larger and we need to be able to see inside the tube. Is it coming out of an old electrical socket?

That looks quite bad. If it gets worse when you use water then there must be a leak somewhere. The roof may also be leaking where the chimney was blocked off. Can you access the space from above to look to see if there are water pipes anywhere or any puddles of water forming?
